You will need a VALID Government Issued ID (Showing Date Of Birth & Full Name)
It is also something that I would want to inform you that you need to get 2-3 photos in total...
1st Photo: Front of the VALID Government Issued ID (Ensure it is NOT expired)
2nd Photo: Back of the VALID Government Issued ID (The "back" of a passport if used is the ID Number, (Sometimes it is the last page of the passport)
3rd Photo: Model Holding the ID next to there Face. See the below photo.
Ensure that all the photos are clear and not blurry.

What the 2 documents that are needed EVERY Time for EVERY Shoot, for EVERY One, serve are required by 18 U.S. Code § 2257 Record Keeping Requirements.
A 2257, Verify's each performer is of age at the time of filming.
A Model Release provides, written model consent / release for content to be distributed and download.
To also provide you some further info for 2257's... They fall under Federal Jurisdiction, what that means is that FBI agents can enter a producer’s place of business without notice and demand to inspect 2257 records under penalty of federal prosecution.
With that being informed you want to keep 2257's / Model Releases separate from everything that you have. Failing to keep 2257 files clean and separate from all other business documentation could result in a waiver of important constitutional rights to privacy if produced to federal agents during a 2257 inspection.
